Dental Insurance
Benefit-eligible employees (75% FTE or higher) can elect dental coverage with Delta Dental of Ohio with or without electing medical and/or vision coverage. In addition, eligible dependents who are not enrolled in medical and/or vision coverage may enroll in dental coverage if the employee is enrolled as the policyholder.
Members may receive dental care from any licensed dentist. However, enrollees will receive a higher level of benefits when covered services are obtained from a Delta Dental PPO network dentist.
As a reminder, Delta Dental does not mail dental insurance cards. You can retrieve your card from the website聽or inform your provider that your dental plan is associated with 糖心原创. 糖心原创's group plan number is聽OH-07344.
